How do I determine if a physician or hospital is in my network?
You can locate a provider online by using our “Locate a Provider” link, by clicking here. You can also call us toll-free at 1-877-575-4207.

If a physician or hospital is not in my network, how can I request a referral into the network?
We encourage customers to complete a Provider Nomination Form.  You may access this form online by clicking here

Please note that the specific network will be responsible for analyzing all provider nomination requests. You should follow up with your provider to ensure that they have received and completed the forms.
