This information is provided in your certificate booklet/policy in the prescription drug benefit provisions. You may also call Express Scripts at the number printed on the back of your prescription ID card.
The address where you receive your prescription ID cards can only be provided to ESI by CGI. But you can specify and change the address where your mail order prescriptions are sent. To begin using the mail-order service, complete a profile/order form at www.express-scripts.com and specify where you would like your medications shipped. If you have not submitted any prescriptions to ESI's mail-order service, they will not yet have a profile or address established for you.
CGI participates in Express Scripts' Drug Quantity Management program. The program places quantity limits on certain medicines, which means you may only receive an amount to last a certain number of days. The clinical criteria for determining the quantity limit is based upon FDA-approved dosing guidelines as stated in the manufacturer's product package insert as well as other medical literature.
Under certain circumstances, exceptions are considered to fill a quantity over the limit. Your pharmacist can contact Express Scripts about getting a prior authorization for a larger quantity or you may contact CGI.
